Home care support for heart attack and cardiac surgery recovery, congestive heart failure (CHF), and other chronic cardiac diseases. If a parent or other loved one is recovering at home from a heart attack or cardiac surgery or living with congestive heart failure (CHF) or another chronic heart disease, Care Plus can connect you with prescreened, reliable cardiac home caregivers who understand the special challenges posed by heart disease. The cardiac home caregivers we can refer to you have the training and experience to provide the appropriate assistance in the comfort and familiarity of your spouse, parent, or loved one’s own household. Personal home care services that caregivers referred by Care Plus commonly provide for individuals who are recovering from cardiac-related surgery or illness and those whose activity is restricted by chronic heart conditions include:

Personal care assistance with activities of daily living (ADLs) such as hygiene, bathing and 
grooming; dressing; transferring, positioning, and mobility/walking; using the toilet/incontinence;
eating
Personal care assistance with instrumental activities of daily living (IADLs) such as meal
planning, cooking, light housekeeping, laundry, pet and plant
care,transportation,errands,shopping, doctor and other appointments
Companionship – encouragement, interpersonal cognitive stimulation, facilitation of physical
activity, and intervention for loneliness and depression

Cardiac personal home care services can be provided on a part-time basis—typically 4 to 6

If medically necessary skilled nursing care for a loved one with cardiac health concerns is
required, Care Plus also refers skilled licensed practical nurses (LPNs) and registered nurses
